Cooperative Annual Performance Report (CAPR) Form

The Cooperative Annual Performance Report (CAPR) is a cooperative reporting instrument that consolidates the requirements currently contained in the Annual Report (AR), Inspection Working Paper (IWP) and General Information Sheet (GIS).

The CAPR applies to all types and categories of cooperatives nationwide.The accomplished CAPR shall be submitted to the Central Office or Extension Offices that has jurisdiction over such cooperative.

Name Reservation Form

The form is used to reserve a preferred name for the cooperative you are organizing. The form should be filled in and submitted to the nearest CDA regional, provincial or city office. A one time processing fee is levied for each name reservation.

Express Lane Registration Guide

It is a program which intends to facilitate and create an efficient system of registering cooperatives with the Cooperative Development Authority (CDA). Registration under the Express Lane involves a 2-hour process-release of Certificate of Registration using Express Lane Forms, counted from the receipt of complete documents by the CDA. Express Lane Registration forms are available for a fee at the Registration Section of the CDA Extension Offices nationwide.
